That there was some type of disconnect or communication issue isn't where I have a problem. It's assuming that the "disconnect" means that McVay wasn't listening or was dictating to Goff. There are NO reports on those two specific issues so to be honest that's speculative. An assumption. Guessing. Using your senses, which may or may not be accurate, to interpret.
