It's possibly that Goff is reading things fast enough, just not getting the right answer. Or at least not getting the same answer McVay is.
To illustrate what I mean: Picture Goff and McVay sitting in front of a screen with a buzzer in their hand. They are asked to watch a play develop and press the buzzer when they recognize the defense. They both hit the buzzer at half a second ... and McVay says Cover 2 while Goff says Quarters.
Did Goff process fast enough? Yup.
Are Goff and McVay on the same page? Nope.
I think that's all semantics. The complaint is that at the end of the day, there is something wrong with Goff's head.
Whether it's not processing fast enough, or processing fast but with the wrong read, or not going through progressions. Bottomline he's not getting it done above the shoulders.
It all results in a QB who is not making good decisions and is ineffective.
