From the CDC directive titled "Responding to Coronavirus (COVID-19) in Nursing Homes" [www.cdc.gov]

The CDC required nursing homes to establish "a designated COVID-19 care unit for residents with confirmed COVID-19". This means that when one or more of their clients came down with COVID-19 those clients had to be physically separated from their other clients, have COVID-19 staff that only cared for COVID-19 patients, and required those COVID-19 staff to use PPE.

Any patient coming into a nursing home as a 'COVID patient' whether they were coming from a hospital, their home, or any place else went into the nursing home's COVID ward without interacting with healthy patients. This is from the same CDC directive: "Newly admitted and readmitted residents with confirmed COVID-19 who have not met criteria for discontinuation of Transmission-Based Precautions should go to the designated COVID-19 care unit."
