I don't suffer from insomnia so I can't imagine what that must be like to want to go to sleep.. and you can't. I am usually wired even when I'm tired and it's rare that I can go to sleep as soon as I crawl into bed. My body is tired but my brain says "I'm not!". A typical radical cystectomy surgery is 5-6 hours. If there are no complications, you'll be in the hospital 5-7 days. You'll be really sore after the surgery but walk, walk, walk every chance you get while you're in the hospital, even while hooked up to an IV. Walking will wake up your bowels.
