More often than not. No matter the opponent. Blocked punt = FG (which we lost by). Goff INT = FG (which we lost by). Webster not being able to fake out the punter - the freaking punter! - in the open field takes 6 off the board and, can't remember exactly, but was that the drive that we passed up the long FG on and turned over on downs?
It's not so complicated. We had problems with players executing. Game plan was good enough to win - players did not do their jobs. Don't hold on the second level, block EVERYONE rushing the passer, don't pass of a back with no coverage on a screen pass, and on and on.
Not every bad team goes 0-16. Jets probably should have got their win against the Raiders - another decent team the Jets outplayed. You have to bring it every play every week and the great teams find a way to do that. I don't think anyone thought this year's Rams would be great. We are still young by NFL standards and have some holes that have been largely masked by, gasp, good coaching. Except ST's - they are horrible and really shouldn't be.
The fact that McVay has gotten this far this year with inconsistent line play (look up the rankings for our O-line), RB by committee, a QB who seems to have regressed (sorry, but he has) to me means he's a great coach. Same for Staley - he has Donald and Ramsey and some good corners and one safety. The other safety is a rookie and makes a lot of mistakes, the guy (Rapp) who was supposed to be there, and is a legit playmaker, is apparently made of glass. We have one functional edge rusher who's hit and miss (Floyd) and inside LB'ers that would be depth players on most teams. That is what you call good coaching.
