One of our Buffs was involved in training that involved long flights out West and up to Alaska. When the plane touched down at Nellis AFB, Nevada, two wheels blew at the same time. One of the Crew.....I believe the tail gunner.....was half asleep. He heard two loud booms and pulled the ejection handle. B-52s had hatches that required SOME forward speed. He just barely clipped the hatch but enough to kill him. I believe that was the only time such an incident happened on a B-52. Not sure what was done but I KNOW within months, that scenario was corrected to never allow that to happen again.
