I'm thinking that those side arm throws.....

are the crux of his elbow problems. All these arm adjustment throws over his 12 years have done damage to his elbow tendons. He'll play through it, he's that type guy, he's been doing it for several years I assume, but with little to no complaints because it's part of his game.

Watch out Mr. Mahomes...you could be next as the years pass.
