Actually, I think it is pretty simple incompetence. Most people plan only for success and do not consider failure. Most coaches don't spend the time to see things from the opponents point of view and consider how another team might attack. What weaknesses do they seek to exploit? What deficiencies will they aim at leveraging?

It's mind boggling to think that this kind of thinking does not occur.
