Remember 2016?? There were several games where Gurley was an absolute NO SHOW. YES, it was under Fisher and a crap team but....... I made my first trip ever to NYC (Thanks to a USAF bud) and while there, saw the Rams play the Jets in 2016. Gurley flat out looked lie he did not give a rat's backside about playing. We sat 5 rows from the field and I watched him closely. There was no signs of pain or injury but his effort.....To be honest???...SUCKED! I'm not saying it's my final answer but just very questionable things I saw in N.Y. The ONLY real highlight that day though was a BOOOOOOOOMING punt by Heckker. We won but it was an UUUUGLY game. I still tend to think it was MCVAY that dropped the ball vs Chicago, Philly and N.E.
But never forget....Those three games, the Rams played like absolute S*** but STILL were not that far from victory. We ALL may be overreacting.
This gets downplayed or neglected by many, but Gurley was physically not himself in 2016 and it showed. Last year McGinnis even said he was not playing healthy in 2016.
So no it's not in his head.
About 2019---this from a recent article: You do not say things like this UNLESS there is a real problem:

The Rams currently have Gurley on an offseason regimen in hopes of getting him completely healthy for the 2019 season, but if that doesn’t work or if his knee regresses, the team may consider stem cell treatment for their star running back, according to a team source.
...
At the time, the Rams attributed Gurley’s absence to soreness and inflammation. According to McVay, the team is hoping that they can get Gurley back at full strength this offseason.
“We’re going to implement a plan to have him attack it to get that knee back to feeling full strength, feeling good based on just the amount of work that he’s gotten,” McVay said. “That would be no different whether he missed any games or not, though.”
They are working on it as a real physical problem.
And your knee does not inflame if the issue is psychological. That's something physical and real.
There are many possibilities as to why (1) a knee would inflame and be sore, and (2) YET it's not a surgical issue (and we know this because they did an MRI)
