If Donald signs an extension this is what would count against the 2018 cap:

His 2018 salary.

A portion of his signing bonus, depending on the length of his contract. Divide the signing bonus by the number of years: that's how much of the signing bonus counts each year. (Note: the bonus can't be spread over more that 5 year so if Donald signs a contract for 6 or more year you still divide by 5.)

Any bonuses paid this year. I can't imagine AD would have a roster bonus or anything like that, but if he did it would count.

Any 'likely to be earned' incentives. i.e. if there's a clause that pays Donald $5M if he gets 3 or more sacks that $5M counts against the cap day 1 (since that incentive is likely to be earned). As opposed to a clause that pays Donald $5M if he gets more that 20 sacks: I'm sure that incentive would be ruled 'not likely to be earned' so it wouldn't count against the 2018 cap. (But if AD did get 21 sacks the Rams would still pay out the $5M, and that $5M would count against the 2019 cap.)
