Insanely rare. Those are all good examples and I'm having a hard time coming up with one of my own. I think the TV series "The Expanse" is the best example I can come up with for TV.
The worst example (which there are too many to list) for me by far is "World War Z". Literally the only thing the book and film have in common is the title. Absolutely nothing from the book is in the film, nothing.. nada... zilch.
