Jimmy, that was all photoshop..

I drew a selection around the arm/hand, copied it and pasted it onto another layer, made a mask for the layer and then stripped the background around the arm/hand.. Then moved that arm/hand down lower - to where you see it..

I then copied a section of his shirt (because the arm is longer, you need shirt material to extend the arm) and pasted that onto another layer, made a mask and stripped the background and positioned it (including distorting it to fit the size of the arm/hand) into place..

Then used the clone tool to "fill in the gaps" on the original photo so that it all looked proper..

Then created another layer and sampled the shirt color etc and air brushed everything to look smooth..

Total edit time was about 5 minutes.. Normally you like to work with a HUGE resolution photo, but sometimes you just hafta work with small photos, so the result isn't as dramatic...
