Like you, I don’t have any background or expertise regarding mental health.
My best guess:
For those of us who got married and/ or had children one day we all stumble across the revelation that life is much bigger than just us. McVay’s wife being from Ukraine and their fear and uncertainty for her family so shortly after the SB might have been the first time McVay realized there is more to life than football. That is shortly followed by an unprecedented challenging season. After the SB more coaching changes, losses of team leaders like Whit and Woods, failures in adequately replacing exiting coaches and free agents, and the absolute ridiculous amount of injuries all season. Lastly and possibly most importantly, the loss of his grandfather this season. McVay for years has attributed all of his coaching opportunities, and successes to his grandfather, whom he has mentioned in pressers that he would call to help him work through football issues.
My point is that you have a young man that likely had a myopic view of the world, who reached the pinnacle of success very early in his coaching career only to be met with the realities of life shortly afterwards. Marriage, losing loved ones, coworkers and friends moving on, failure leading to self doubt, and questioning your ability to bounce back and have the same successes you once were able to reach, are all things that some of us will face during life.

I don't know anything about mental health, but from listening to McVay here it sounds like he had an episode of some kind. Maybe a major anxiety attack or a minor breakdown
Just speculating based on his answers and phrasing. But again, I'm out of my wheelhouse on this subject.
