When, in the game, does playcalling go flat? And why?

You guys have seen the games - with the exception of two, I haven't.

From my distant and all but blinded view, the playcalling shifts somewhere through the game and goes to a narrow regurgitation of what worked with different personnel last year, with unrealistic expectation that players who aren't up to it somehow execute.

Is that because (you can only fool the D with so many jet sweeps) we run out of plays? Or is the game plan flawed? Or is there a failure to adjust at halftime? Or because we're playing to protect a lead? Or is it because we run only in first quarter (sometimes it takes incesssant pounding to establish the run) and abandon it prematurely?

All of these possibilities have been floated here on this board - what is happening, really?
