You really try NOT to do surgery. The outcome is so uncertain. Injections, up to a point, and physical therapy with periods of rest, are the treatments used. Some have used iontophoresis (electrical stimulation driving substances under the skin into the tendon) with steroids or other chemicals, but they haven't been shown to be any better than injections.
