It's not complicated. It's just the same thing McVay does except on Defense: Show the same basic formation on every down then before the snap change to the real formation.
Except McVay likes to make his offensive call based on the defense moments before the helmet radio shuts off. He can't, so it falls in Goff's lap, and Goff can't read defenses fast enough before the play clock runs out.
The way to combat is to play hurry-up and stick with the run.
Yes this wait until the last minute in the play clock game is ridiculous and the reason we burn so much unnecessary time outs. I get it that mcvay is smart and wants to outsmart everyone but it takes its toll sometimes. Go out and run the plays that work in practice and games and trust that your guys will be in the right spots and make the right adjustments. Plays are succesfull because of great execution not just because they caught the defense in the right alignment.
