Many things could cause inflammation and result in Gurley being day-to-day.
There could be an excess build-up of scar tissue, or moderate cartilage damage, or a mild meniscus tear. A sprain is only one of several possibilities.
How is it that a mild tear is day to day? And how is moderate cartilage damage day to day? Wouldn't both require more extended and definite recovery if they didn't require surgery? There aren't even reports of him wearing a brace. (Though that could change.) And how would a sudden trauma that occurred on the field lead to a build-up of scar tissue immediately like that?
Anyway that's why my "on a message board" guess is a sprain. There were reports they were optimistic he could play, at least up to the last minute. So I guess a sprain because near as I know those other things would require more definite time.

The short answer is... because the human body is complicated and mysterious, and on top of that, football teams are (understandably) secretive about injuries.
Minor cartilage damage can (sometimes) heal up well enough on its own, sometimes quickly, sometimes slowly, sometimes never.
A small meniscus tear... same thing. Depends on the person and the precise location of the tear.
All I know is, usually when it’s a sprain, they say so, but in this case they keep using the more vague term “inflammation”.
Lastly— the doctors themselves may not know for sure. Minor injuries are difficult to diagnose, sometimes they only know what the problem is after they do a scope and actually see what’s going on. I don’t think we fans will find out the truth until the offseason and gurley has had a scope.
