Teams do a lot of different things with their staffs sure. But IMO if McVay felt good about his passing game coordinator production he wouldn't have hired an OC. Or he would have promoted the passing game or run game coordinator to OC.
I also think the fact neither of them were promoted to OC last year is a great example of how smart McVay is in spite of his relative youth as a head coach. Lot of young head coaches would have filled the job. McVay left it open and we saw this season why he did that... Because neither of those guys were up to snuff.

You keep a qb coach who is not "up to snuff"? McV left a job open he could have filled...a job which, when he filled it, it was because he needed to, yet he left it empty before? Kromer, rest assured, will still be run game coordinator because whether or not they officially have the title, OL coaches contribute to the running game.
I don't think failing to fill a position you learn the hard way you needed filled is an indication of how smart you are. Plus, again, the coordinator last year was McVay himself. He's replacing himself.
I would worry if I thought for some reason McV was retaining a coach he didn't believe in, especially a coach for an important role like qb position coach. Since I have no reason to believe that;s what happened, I am not so worried.
Odds are there's less drama here than people are looking for. Just my 2 cents.
