McVay adjustment issues

McVay seems to be able to build a team culture really well, he seems to be able to construct and install an offense really well. He does not seem to adjust well.

Be it 2018 or this season, what we see are defenses figure us out and it becomes game over. An explosive offense gets tamed. It seems it takes a really long time for McVay to craft an offensive response that is sufficient, but once that is complete defenses adjust again.

Other ways adjustments seem to not come in the moment. Koski, why did he continue returning? Swify decision to fire coaches happen in the off season but have yet to happen in season. Once the season starts we have what we have. This concerns me as it seems a skill needed to win a superbowl.

I also am not sure if McVay can grow in this area. It seems to not be in his wheelhouse and has yet to show up. This is why he is undefeated while leading at half, but loses a huge majority of those games trailing at half. The former games go the way we planned, but the latter involve defense that is disrupting. Half time adjustment skills would lead to more comebacks, but we just never comeback.
