I just watched a replay of that one against the Dolphins last year. I've seen QBs make throws like that before where they are rushed and just don't see a DL. Carson Wentz was looking right at the 49ers player (there wasn't even anyone else in the area) when he threw it to him.
The ball was tipped as it left Wentz' hand. Doesn't excuse a terrible decision, though.
