They can but they don't all do that. Demoff doesn't typically back load his contracts much. Sometimes he structures a lower first year number, sometimes he doesn't.
If Ramsey signs for close to $18M I wouldn't count on having any salary cap savings in 2020.
If he signs in 2020 my bet is they do what they did for example with Donald--the year of the signing (in this case 2020) remains the same, and the new contract starts the following year (in this case 2021). In which case the extension has no effect on the 2020 cap.
To back you up on frontloading, yes the Rams can do that, though the big frontload hit isn't always in the 1st year of the contract. For example Goff;s deal counted as 36+ M in 2019, but counts 26 M against the cap in its final year (2024).
Goff was also the same as Donald and what I am saying will probably happen with Ramsey. In 2019 he was only 10+ M against the cap. Basically the extension starts in 2020 even though he signed it in 2019.
