There's no way to know if it's Case having a tendency to lock on based on his pre-snap read and trying to force it in there. Wouldn't surprise me though and I'm sure at a minimum that's part of it.
But even with that I believe the staff, starting from Fish, is pushing to get the ball into Tavon's hands and it's the wrong approach because he's not that type of receiver, the guy with the catch radius and velcro hands. Let the QB run his read and his job by dealing that ball where he thinks it needs to go, not have concern in his head about "Man I haven't gotten it out there to Tavon in the past six throws." Not saying that's what happened on that particular play you list, but I do think it is a factor.
