It has to be a combination of things...One...I think McVay's personnel and play calling decisions come into questions and as you point out, it seems Goff seems to miss the target. For example...Kupp against a MLB? Is there any decision but to throw it over the top...Kupp has a stride and the MLB has his back turned.
Just watch the next few games. I'm telling you...we struggle inside the 30-20 when we have a first down. Yet, when we get inside the 15 and have a first down, we tend to finish off the drive.
