He simply got stuck in a narrow passage? This occurred at the beginning of the rescue attempt and I remember reading that they had expanded some of the passages.... possible explosives, so I am wondering if it wasn't so much a malfunction or miscalculation as it was an accident.
I can well imagine that the rescuers are working so frantically that 'reporting' is far from a priority right now and the media is probably forced to go with 'tidbits' which may not be 'wrong' but an incomplete picture. - JamesJM
