It looks to me like Cooks was not the first read as Goff isn't looking his way.
Problem with that is that reads should not be static. First read should be dynamic depending on what the defense does post snap. A QB can't be in the huddle and say no matter what happens I'm looking at you first.
Should be something like.. if the def plays cover 2 zone my first read is the post then the corner. Or if the def blitzes then I'm looking at my blitz beater first.
Remember Phroel's TD catch against TB. Warner told Phroel in the huddle that if the safety comes up go deep, and I'm throwing to you. If the safety stayed deep, Warner woudn't have even glanced at Phroel.
Once Goff recognizes man in this instance, he should've changed his primary to Cooks on the quick slant out. Knowing he only needed two yards, and especially with the possibility of a pick play.
Good post and you may be 100% right. But we don't know what was called or what the progression was supposed to be. In hindsight it certainly does look like a missed opportunity.
Like I said, it was Goff's worst game of the season. It happens to the best of them from time to time.
