'lack of cohesion' caused by not being on the same page? I cannot. Lack of cohesion due to personality conflicts, sure... and other possible reasons as well... but I can't imagine Fisher, or any other HC, not making sure his 'prep' was structured to make sure things were in sync.

Well I agree more with you.
Here's what I saw last year (and this covers only one thread in the whole carpet).
A first time coordinator installed basically a new offense during a move. As a recipe, that sounds like mix gasoline and 4th of July sparklers and add a lit match.
It showed badly in the 1st 2 games when they were just miserably out of sync.
They did better over the next 3-4 games or so.
BUT if you do not have everything put solidly into the foundation in pre-season, there's only so much fixing you can do during the season.
As I said that;s just one strand. And that's even changing topics from the "what was with Gurley" strand. Which added up is just 2 of many strands.
If there was a lack of cohesion IMO it was between the players. Most of the coaches were the same and it's hard to imagine coaches getting that incoherent with one another.
